1Brain protection of AC in PTZ-induced epileptic rats

Abstract:
Objective: To investigate the neuroprotective effect and mechanism of Acteoside (AC) on brain injury in epileptic rats. Methods: Totally 30 Sprague-Dawlay rats (SPF grade, healthy males, about 200 g) were randomly divided into epilepsy group, AC group and blank control group. The epileptic group and the AC group were given 28 d of intraperitoneal injection of pentaerythritazine (PTZ), and the remaining groups were given normal saline injection. The changes of electroencephalogram (EEG), learning and memory ability, pathological changes of brain tissue, superoxide dismutase (SOD) and malondialdehyde (MDA) were observed in all rats. Results:Compared with the epilepsy group, the AC group showed significantly reduced degeneration and necrosis of nerve cells in brain tissue, significantly improved learning and memory ability, and the EEG was dominated by wavelet and slow wave, replacing the typical spike and spike waves. SOD increased and MDA decreased in oxygen radical (P<0.05). Conclusions: Epilepsy are closely related to Oxidative stress. AC can play a protective role in the brain by improving EEG, learning and memory ability, regulating changes in SOD and MDA content, and inhibiting the generation of oxygen free radicals.

2Study on the correlation between pyroptosis and patients with chronic heart failure

Abstract:
Objective: To study the changes of pyroptosis in heart failure and its significance. Methods: Totally 180 patients with chronic heart failure (the chronic heart failure group) and 40 healthy patients (the normal control group) diagnosed in our hospital from February 2018 to October 2019 were selected. According to the New York Heart Association (NYHA) classification standard, among the 180 patients there were 48 cases of Ⅱ level, 60 cases of Ⅲ level and 72 patients of Ⅳ level. After admission, the N-terminal brain natriuretic peptide precursor (NT-proBNP), the left ventricular end-diastolic diameter (LVEDD), the left ventricular end-diastolic volume (LVEDV)and the left ventricular ejection fraction (LVEF) were examined by ultrasonic morphological examination. The expression levels of NLRP3, caspase-1 and IL-1β in serum of all samples were detected by ELISA, and the association of NLRP3, caspase-1, IL-1β and related indicators in patients with chronic heart failure of different grades was statistically analyzed, so as to investigate the relationship between pyroptosis and the occurrence and development of chronic heart failure. Results: The levels of NLRP3, caspase-1, IL-1β, NT-proBNP, LVEDD and LVEDV in the chronic heart failure group were higher than those in the normal control group and LVEF in the chronic heart failure group was lower than that in normal control group, with statistically significant differences (P<0.05). Levels of NLRP3, caspase-1, IL-1β, NT-proBNP, LVEDD and LVEDV increased with patients'cardiac function grading, while LVEF decreased with patients' cardiac function grading P<0.05). Pearson analysis showed that NLRP3, caspase-1 and IL-1β were positively correlated with NT-proBNP, LVEDD and LVEDV (r=0.398, r=0.396, r=0.398, r=0.506, r=0.499, r=0.505, r=0.449, r=0.422, r=0.424; P<0.01) and negatively correlated with LVEF (r=-0.400, r=-0.555, r=-0.438; P<0.01). The area under the ROC curve of NLRP3, caspase-1 and IL-1β for diagnosing heart failure was 0.768, 0.805 and 0.799, respectively, with statistically significant differences (P<0.01). Conclusion: Serum levels of NLRP3, caspase-1 and IL-1β were higher in patients with chronic heart failure than in the controls and continued to increase as the heart function deteriorated. Pyroptosis has a certain guiding significance for the occurrence and development of chronic heart failure.

Abstract:
Objective: To observe the effect of Elephantopus scaber Linn on community-acquired pneumonia and its relationship with the expression of TLR4/NF-κB pathway and the downstream inflammatory cytokines IL-6, IL-8, TNF-α. Methods: A total of 140 patients with community-acquired pneumonia were randomly divided into observation group and control group, with 70 cases in each group. Patients in observation group were treated by Elephantopus scaber Linn + atomized inhalation of normal saline + intravenous injection of ceftazidine; while patients in control group were treated by intravenous injection of ceftazidine; Meanwhile, 70 healthy subjects were selected as the healthy control group. The expressions of TLR4, NF-kB, IL-6, IL-8, and TNF-α before and after treatment were detected by enzyme linked immunosorbent assay. Results: Compared with the control group, recovery rate, total effective rate, disappearance time of symptoms of the observation group was significantly better than the control group (P<0.05). Compared with the healthy control group, the serum TLR4, NF-κB, IL-6, IL-8, TNF-α were significantly higher (P<0.05) in the observation group, which were significantly decreased after treatment with Elephantopus scaber Linn (P<0.05). Conclusion: Elephantopus scaber Linn can decrease the expression of TLR4 and NF-κB in blood and its downstream inflammatory factor. Elephantopus scaber Linn may inhibit TLR4/NF-kB signal pathway to exert anti-inflammatory effect and can be used to treat community-acquired pneumonia.

4Expression and clinical significance of miR-186 in serum exosomes of patients with chronic obstructive pulmonary disease

Abstract:
Objective: To study the expression of miR-186 in serum exosomes of patients with chronic obstructive pulmonary disease, and to explore its clinical significance with chronic obstructive pulmonary disease. Methods: The peripheral blood sera of 53 COPD patients and 53 normal persons in our hospital were collected. Extraction of exosomes and electron microscopy to identify the characteristics of exosomes. RT-PCR method was used to detect the miR-186 expression level in serum exosomes. And statistical analysis was conducted to compare the expression difference between the COPD group and the normal group, the correlation between miR-186 and FEV1% predicted value, and ROC curve analysis of the diagnostic value of serum exosome miR-186 in COPD. Results: The relative expression of miR-186 in serum exosomes of COPD group was 3.24±0.14, which was significantly higher than that of normal control group 1.03±0.04 (t=15.47, P<0.001). The expression level of miR-186 was negatively correlated with the predicted FEV1% of COPD patients (r=-0.502 4, P<0.000 1). The area under the ROC curve between miR-186 and COPD patients was 0.840 9 (95% CI: 0.687 5-0.994 3). Conclusions: The high expression of miR-186 in serum exosomes may be a risk factor and may be used as a diagnostic indicator of COPD.

5Clinical efficacy of Xibitong recipe combined with platelet-rich plasma in the treatment of moderate knee osteoarthritis

Abstract:
Objective: To observe the clinical effect of Xibitong recipe combined with platelet-rich plasma in the treatment of middle staged knee osteoarthritis. Methods: From May 2018 to January 2019, 90 patients with moderate knee osteoarthritis (K-L classification Ⅱ and Ⅲ) which are liver and kidney deficiency type were admitted, and divided into experimental group and control group. The experimental group included 15 males and 30 females,aged 46 to 70 (58.93±10.67) years and the course of disease was 1 to 8 years (27.06±4.22) months, and 33 cases in grade Ⅱ and 12 cases in grade Ⅲ of K-L classification. Xibitong granules fomula was taken orally one pack per day, in the morning and evening respectively, 14 d was considered as a course of treatment, and while platelet-rich plasma (2 mL) articular cavity injection was also given to the experiment group, once every 14 d. The control group was consisted of 12 males and 33 females aged 43-68 (57.00±8.83) years old; the course of disease was 1-8 years (25.40±3.72) months; the results of KL classification are 36 cases of grade Ⅱ, and 9 cases of grade Ⅲ, and the control group were given platelet-rich plasma ( 2 mL) injection into the joint cavity only, the treatment course and frequency are the same as the experimental group. The VAS score, WOMAC score, matrix metalloproteinase-1 and interleukin-1β levels of joint fluid before and one month after treatment were recorded and compared between the two groups. Results: Follow up on the 90 cases showed no serious adverse reactions or other complications. The WOMAC scores of the experimental group and the control group before and after treatment were (106.40±47.96), (116.13±41.15), (55.13±22.02), and (62.67±26.23), respectively. The VAS scores of the two groups before and after treatment were (5.87±2.45), (5.80±2.15), (2.67±1.35), and (4.00±1.96), respectively. The levels of interleukin 1β in the test group and the control group before and after treatment were (78.33 ± 7.23), (76.60 ± 7.07), (25.73 ± 3.24), and (31.13 ± 5.74). The levels of matrix metalloproteinase-1 before and after treatment in the two groups were (2 299.66± 79.34), (2 206.66±194.24), (1 741.86 ± 183.08), and (1 762.46 ± 192.10). Conclusion: For patients with moderate knee osteoarthritis, the combination of platelet-rich plasma and Xibitong recipe shows better therapuic effects than platelet-rich plasma alonein relieving knee pain, improving knee function and reducing the levels of interleukin-1β and matrix metalloproteinase-1 in joint fluid. And the side effects are small, it is worth clinical application.

6Effect of Tongluo Shenggu Prescription on ARCO Stage II non-traumatic femoral head necrosis: A prospective study

Abstract:
Objective: To observe the clinical efficacy of Tongluo Shenggu Decoction in the treatment of ARCO Ⅱ non-traumatic femoral head necrosis. Methods: A total of 120 patients with ARCO Ⅱ non-traumatic femoral head necrosis from January 2016 to January 2017 were included in the paired control prospective study. These 60 cases in the treatment group were treated with Tongluo Shenggu Formula. The control group (60 cases) received high energy extracorporeal shock wave therapy. The imaging stability rate, Harris score, and SF36 score were used as clinical efficacy indicators. All cases were followed up for 24 months, and the follow-up results were statistically analyzed. Results: Among these 120 patients, 5 patients were lost and the rest were followed up. The imaging stability rate was high, with a similar result between the two groups (78.95% in the treatment group and 72.41% in the control group) (P>0.05). The Harris score of the treatment group was 92.67±8.45, significantly higher than that of the control group (80.39±24.65) (P<0.05). The pain degree, joint function, and joint deformity score of the treatment group were better than those of the control group (P<0.05). The SF-36 score of physiological function, physiological function, physical pain, overall health, social function, and emotional function in the treatment group were significantly higher than those in the control group (P<0.05). The incidence of adverse events of the two groups was low and all patients were relieved later. Conclusions: Tongluo Shenggu prescription has a definite curative effect in treating ARCO Ⅱ non-traumatic femoral head necrosis, especially in improving hip joint function, relieving hip joint pain, and improving quality of life.

7Application of individualized nutritional intervention therapy in patients with gestational diabetes mellitu

Abstract:
Objective: To evaluate the efficacy of individualized nutritional intervention therapy in patients with gestational diabetes mellitus (GDM). Methods: A total of 80 GDM inpatients admitted to the First Affiliated Hospital of Hainan Medical College from December 2015 to June 2018 were selected and divided into 4 groups according to the random number table method, with 20 cases in each group. Food exchange portion combined with glycemic index was used in group 1, food exchange portion combined with blood glucose load in group 2, glycemic index combined with blood glucose load in group 3, and both glycemic index and blood glucose load were used based on food exchange portion in group 4. After intervention, the changes of blood glucose level, biochemical indexes and fetal growth and development indexes of the four groups were determined. Results: After nutritional intervention, the blood glucose levels of the patients in four groups were decreased to varying degrees compared with those before the intervention with statistically significant difference (P<0.05). In order to evaluate the advantages and disadvantages of the four methods, the blood glucose levels of the patients in four groups were further compared between groups: group 1, 2, and 4 showed the significant changes compared with group 3 (P<0.01 or 0.05). A significant change in blood glucose level measured 2 h after dinner was observed in group 4 compared with group 1 and 2 (P<0.05). In terms of the biochemical indexes and fetal growth and development indexes of the four groups, the differences were not statistically significant (P> 0.05). Conclusion: Individualized nutritional intervention therapy can significantly improve the blood glucose level of GDM patients, and meet the needs of the mother's own physiology and fetal growth and development. Among all the methods used in this study, the nutrition intervention method that combines the glycemic index and the blood glucose load on the basis of the food exchange portion has a better effect on improving the blood glucose level of the patient.

Abstract:
Objective: To explore the clinical effect of Yinyang blending acupuncture combining with Banxia Baizhu Tianma decoction in treating of cervical vertigo. Methods:A total of 120 patients with cervical vertigo treated in our hospital were selected and divided into acupuncture group and acupuncture combing with Banxia Baizhu Tianmadecoction group according to the time sequence of admission with 60 cases in each group. Both groups were given rountine treatment, based on which, the acupuncture group was treated with Yinyang blending acupuncture while the acupuncture combing with Banxia Baizhu Tianma decoction group was also given Banxia Baizhu Tianma decoction. The left and right vertebral arteries (LVA), right vertebral arteries (RVA), and basal arteries (BA),clinical symptom score, clinical efficacy were compared before and after treatment between the two groups. Results:After treatment, the Vm values of LVA, RVA, and BA in the acupuncture group in the acupuncture combining decoction group were significant lower than those in the acupuncture group (P<0.05).After treatment, scores of dizziness, neck and shoulder pain, headache, daily life work, psychological and social adaptability of the acupuncture combing with decoction group were significant higher than those of the acupuncture group (P<0.05); after treatment, the cure rate of the acupuncture combing with decoction group was 42.37%, the effective rate was 37.29%, the efficiency was 16.95%, the inefficiency was 3.39%. While, in the acupuncture group, the cure rate was 26.32%, the effective rate was 35.09%, efficiency was 29.82%, and the ineffective rate was 8.77%. The clinical efficacy of the acupuncture combing with decoction group was better than that of the acupuncture group (P<0.05). Conclusion: Yin-yang blendingacupuncture combiningwith Banxia Baizhu Tianma decoction is effective on cervical vertigo, and it can effectively alleviatethe clinical symptoms and improve the therapeutic effects of the patients.

Abstract:
Objective: To explore the diagnostic value of mammography and MRI in breast intraductal papillary carcinoma (IDPC) and compare the diagnostic value of the two methods. Methods: Collected 28 IDPC patients who underwent mammography and MRI from March 2011 to June 2019 and were confirmed by surgery and pathology. The imaging manifestations were analyzed and the accuracy of IDPC diagnosis was compared between the two methods. Results: Mammography of mammography: 24 cases showed masses, 3 cases showed asymmetric dense shadow with calcification, 1 case showed large duct dilation in the areola area, and 6 cases showed short burrs on the edge of the tumor. MRI scan: 28 cases of lesions had low signal intensity on T1WI and high signal on T2WI. 19 cases showed cystic solid masses with small nodules on the cyst wall. 25 cases showed obvious uneven enhancement lesions. The diagnostic accuracy of MRI was 89.3%. (25/28), the accuracy rate of mammography Xray examination was 75.0% (21/28). There was no statistical difference in the diagnostic accuracy between the two methods, and it was not statistically significant (P=0.29). The combined use of the two inspection methods has a diagnostic accuracy rate of 96.4%. The combined two methods are more valuable than relying solely on mammography (P=0.03). Conclusion: IDPC is characterized by lobular or round masses. Large cysts with small nodules may be a special MRI manifestation of this cancer. Combining mammography and MRI can further improve the diagnosis of IDPC.

10A Meta-analysis of Ginseng combined with conventional therapy for stable angina pectoris

Abstract:
Objective: To evaluate the efficacy of Ginseng combined with conventional therapy for stable angina pectoris(SAP). Methods: From the Cochrane Library, Pubmed, Embase, Web of Science, CNKI(China National Knowledge Infrastructure), Wanfang Datebase, VIP(Chinese Scientific Journals Database), CBM(Chinese Biomedicine Database), we reviewed the clinical randomized controlled trial(RCT), after screening and assessing the risk of bias, used RevMan 5.3 and Stata 15.0 software to conduct the meta-analysis. Results: Twelve studies were included with 1 176 cases, involving 606 cases in the experimental group and 570 in the control group. The results of meta-analysis showed that Ginseng combined with conventional therapy significantly has obvious effect on clinical effective rate(RR=1.29, 95% CI[1.21, 1.36], P<0.000 01), ECG effective rate(RR=1.35, 95% CI[1.22, 1.50], P<0.000 01), number of angina attacks(MD=-1.77, 95% CI[-2.64, -0.91], P<0.000 01), duration of angina pectoris(MD=-2.16, 95% CI[-2.54,-1.78], P<0.000 01), nitroglycerin dosage(MD=-1.52, 95% CI[-1.81, -1.23], P<0.000 01), and it is better than using conventional therapy alone. Conclusion: Ginseng combined with conventional therapy for SAP can significantly improve clinical effective rate and ECG effective rate, reduce the number of angina attacks, shorten the duration of angina pectoris, and reduce nitroglycerin dosage. The development of ginseng-related proprietary Chinese medicines has good prospects. But due to the quality of studies is medium and low, it still needs to be confirmed by conducting high-quality RCTs.

Abstract:
Objective: To explore the molecular mechanism of Buzhong Yiqi decoction in the treatment of senile functional constipation based on network pharmacology. Methods: Chemical compositions and selected targets related to the eight traditional Chinese medicine herbs were searched through the Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform. Through GeneCards database and OMIM database, disease targets of senile functional constipation were searched. R language was used to screen the common targets between drugs and disease, and then the interaction network diagram of the targets was constructed by String. Cytoscape3.7.0 was applied to construct the traditional Chinese medicines-chemical compositions-disease targets network. GO enrichment analysis and KEGG enrichment analysis of the targets were performed based on DAVID. Results: Five main chemical compositions including quercetin, kaempferol, nobiletin, naringenin, and formononetin were screened, and five key targets including PTGS2, ESR1, AR, NOS2, and PPARG were identified. 622 GO functional entries and 125 pathways were yielded by gene enrichment analysis. Conclusion: Buzhong Yiqi decoction may improve senile functional constipation via anti-inflammatory and anti-oxidation effect with multi-components, multi-targets, and multi-channels.

Abstract:
Objective: To explore the medication rules of Professor REN Shun-ping’s prescription for treating spleen deficiency and qi stagnationsyndrometype of functional dyspepsia, so as to provide reference for clinical prevention and development of new drugs. Methods: The 144 Chinese herbal formulas considered as active prescribed by professor REN Shun-ping for patients with spleen deficiency and qi stagnation syndrometype of functional dyspepsiawho visited for the first time were collected, and entered into TCM inheritance auxiliary platform (V2.5), and then mined by some data mining methodsof the software, such as the association rulesand unsupervised entropy hierarchical clustering. Results: This study involved 144 Chinese herbal formulas, including a total of 56 Chinese herbs. Of these Chinese herbs,the herbs of cold and warm nature,and pungent, bitterand sweet in flavors were used in a quite high frequency, all of which act onspleen, stomach, lungs, large intestine, liverand gallbladder meridians.Based on the association rules analysis results, a core Chinese herbal formula was summarized which is composed of patchouli, perilla stem, radix bupleuri, scutellaria, ginger pinellia, codonopsispilosula, magnolia bark, tangerine peel, large-headed atractylodes, immature bitter orange, wood incense, fructusamomi, inulabritannicachinensis, ginger, medicated leaven, fried rice and fried malt. And a total of 17 groups of core herbs and 2 groups of new Chinese herbswere discovered based on unsupervised entropy hierarchical clustering. Conclusions: In treating spleen deficiency and qi stagnation syndrometype of functional dyspepsia, professor Renmainly treats spleen deficiency and qi stagnation syndrometype of functional dyspepsia from the perspective of "tongjiang", using the "five-in-one" strategy of strengthening the spleen, evacuating the liver, coming down qi from the stomach, dispersing lung, and bowel-reflexing to treat patients as a whole. And dosage of prescribedherbsare light, herb nature is mild. Atthe same time, heprescribeswith great diversity based on comprehensive consideration of syndromesandsymptoms.

Abstract:
MicroRNA (miRNA) plays a key role in the molecular regulation of neurological diseases, and the molecular mechanism of miRNA is closely related to the occurrence and development of Hypoxic-Ischemic Encephalopathy. Recently, it has been reported that various miRNA molecules can inhibit target mRNA and even degrade target mRNA by changing the complete complementary or incomplete complementary binding to the target mRNA. Therefore, miRNA is of great significance for the study of mRNA related to HypoxicIschemic Encephalopathy. This article briefly reviews the molecular mechanisms, functions and regulation of miRNAs on Hypoxic-Ischemic Encephalopathy.
